Hygiene Instrument Glossary

Gracey or universal? After Five or Mini? UNC 15 or WHO? Hand instrument terminology encodes real clinical differences. This glossary defines the anatomy, patterns, and technique terms behind the hygiene instruments in our catalog, from curette blade geometry to probe graduations.

Instrument Anatomy
Working End The business end of the instrument — the blade of a curette, the tip of an explorer, the graduated shaft of a probe. Double-ended instruments carry a working end at each end, usually a mirrored pair.
Blade / Cutting Edge The sharpened portion of a curette or scaler. Universal blades carry two usable cutting edges; a Gracey's offset blade is used on its lower edge only.
Toe The rounded end of a curette blade — the feature that lets it travel subgingivally without lacerating tissue. Sickle scalers end in a point instead.
Terminal Shank The straight section directly behind the working end. Its length and angle define the instrument's reach; keeping it parallel to the tooth surface is the alignment cue for Gracey technique.
Functional Shank Everything between handle and working end, including all the bends that deliver the blade to its target area.
Rigid Shank A thicker, stiffer shank variant for moderate-to-heavy calculus; standard (flexible) shanks transmit better tactile information for fine deposits.
Double-Ended (DE) An instrument with paired working ends — typically the mirror-image pair of a pattern (e.g., Gracey 5 and 6) on one handle.
Curette Patterns
Gracey Curette The area-specific curette family: each numbered pair (1/2 through 17/18) is bent for a region of the dentition, and the blade face is offset so only the lower cutting edge works. Designed by Dr. Clayton Gracey for subgingival scaling and root planing.
Universal Curette A curette whose blade face sits at 90° to the terminal shank with two working cutting edges — one instrument covers all surfaces. Columbia, McCall, Barnhart, and Langer patterns are universals.
Langer Curette A hybrid: Gracey-style area-specific shank bends carrying a universal (two-edged) blade — region-specific access without blade-orientation rules.
Columbia Curette Classic universal pattern (2R/2L, 4R/4L, 13/14) with a short, curved, slightly broad blade; the 13/14 is a full-dentition workhorse.
McCall Curette Universal pattern (13S/14S, 17S/18S); the S versions carry pointed-toe blades for interproximal access.
Extended Reach / After Five Variants with a terminal shank roughly 3 mm longer than standard, for pockets of 5 mm and deeper.
Mini / Micro Mini Variants with a shortened (about half-length) and often thinner blade on an extended shank — for narrow pockets, furcations, and line angles.
Scalers
Sickle Scaler A pointed-tip scaler with a triangular cross-section and two cutting edges, for supragingival calculus. The pointed tip is why sickles stay above the gumline.
Anterior Sickle Straight-shank sickles (H6/7, H5/33, Morse 0/00) for incisors and canines.
Posterior Sickle Contra-angled sickles (204, 204S, 204SD) whose bends reach around premolars and molars — the 204S is the classic.
Jacquette Scaler A straight-bladed sickle family (30/33, 31/32, 34/35) with flat faces.
Towner Scaler The U15 pattern, commonly paired as U15/30 or U15/33.
Contra-Angle A shank with opposing bends that offsets the working end from the handle axis — the geometry that makes posterior access possible.
Explorers & Probes
Explorer A fine, flexible, sharp-tipped diagnostic instrument for detecting caries, calculus, and margin defects by tactile feedback.
Shepherd's Hook (#23) The classic single-curve explorer shape and the standard caries-detection tip.
ODU 11/12 Explorer The Old Dominion University pattern: paired, extended contra-angled tips that adapt subgingivally around the full dentition — the calculus-detection standard.
Cowhorn Explorer (#3CH) A curved, paired explorer for interproximal and furcation exploration.
Periodontal Probe A blunt, graduated instrument for measuring pocket depth, recession, and attachment loss. Defined by its marking pattern rather than a shape name.
UNC 15 (North Carolina) Probe marked every millimetre from 1–15 with bands at 5, 10, and 15 — the periodontics standard. Helmut Zepf catalogs it as the North Carolina CNC.
WHO / CP 11.5 Probe The World Health Organization screening probe: 0.5 mm ball tip with marks at 3.5, 5.5, 8.5, and 11.5 mm; used for PSR/CPITN screening.
CP 12 / Marquis-Style Color-coded probe marked 3-6-9-12 mm — fast general-practice pocket charting.
Nabers Probe The curved, blunt furcation probe (usually 3-6-9-12 graduation) for classifying furcation involvement.
Graduation The measurement markings on a probe — the property that defines probe equivalence across brands.
Mirrors
Front Surface Mirror Reflective coating on the front of the glass — a single, distortion-free image. The clinical standard; rhodium coating adds scratch resistance.
Plane (Flat) Surface Mirror Coating behind the glass — economical, with a slight double image.
Mirror Sizes #4 / #5 The universal head sizes: #4 ≈ 22 mm and #5 ≈ 24 mm diameter.
Cone Socket vs. Simple Stem The two head-to-handle attachment systems; they are not interchangeable, so match replacement heads to your handles.
Double-Sided Mirror Mirrored on both faces — retract with the back while viewing with the front.
Zepf Systems & Coatings
M5 (Zepf) Helmut Zepf's deep-pocket curette design: first shaft about 3 mm longer than standard with a shortened, 0.95 mm thin working end — for tight, deep pockets and narrow root surfaces beyond 5 mm. Conceptually parallel to After Five / Extended Reach designs. See the Langer M5 literature.
Nanopal® Zepf's black crystalline nano-coating that raises surface hardness to 4,500 Vickers — cutting edges stay sharp far longer and do not need resharpening after each curettage; the scratch-resistant surface also suppresses light reflections.
bIONIK Handle Zepf's ergonomic PEEK handle (Red Dot design award 2010), available in ten colors used for anatomical color coding, with exchangeable working tips.
QuickFix / Z-SHAPE Relax QuickFix is Zepf's tool-free interchangeable-tip thread system (M4 × 0.5 mm); Z-SHAPE Relax is the ergonomic handle generation designed around the curettage working position.

Technique & Care
Scaling Removal of plaque and calculus from tooth surfaces, supragingival or subgingival.
Root Planing / Debridement Smoothing and decontaminating root surfaces subgingivally — the domain of Graceys and extended-reach designs.
Adaptation Keeping the working end correctly contoured against the tooth as it moves — the skill the pattern geometry exists to serve.
Angulation The blade-to-tooth angle: effective scaling happens between roughly 45° and 90°, ideally 60–80°.
Sharpening Restoring the cutting edge on a stone while preserving the blade's original bevel geometry; a dull curette burnishes calculus instead of removing it.
Tactile Sensitivity The transmission of surface information through the instrument — why shank flexibility, handle weight, and knurling matter.
Interchangeable Inserts Helmut Zepf's TOUCHGRIP system of screw-in (M2.5 / M4 × 0.5 thread) probe and explorer tips — replace the worn tip, keep the handle.
