Perforated Periosteal Elevator

The perforated periosteal elevator is a curette-style periosteal elevator with holes through the blade, designed by Dr. Cyril Niederquell for bone augmentation: it dissects the mucoperiosteum and lets the clinician press bone substitute material firmly into place with the same instrument. The strategically placed perforations in the blade provide grip on the periosteum while reducing pressure on the soft tissue — for efficient, atraumatic flap reflection.

Clinical Use

The instrument is used for full-thickness mucoperiosteal flap elevation in periodontal and implant surgery, bone grafting and ridge augmentation, and lateral-window sinus lift access; once the flap is reflected, the perforated blade doubles as a packer for particulate graft material. The holes reduce the suction effect between blade and tissue, improving grip on the periosteum while reducing tissue compression.

Care and Reconditioning

Periosteal elevators are cleaned, disinfected, and sterilized between patients; brush the perforations during manual cleaning so no debris remains in the holes, and return the instrument for reconditioning when the working edge dulls.
